You are requesting access to portions of your health information and the ability to communicate with your healthcare providers regarding your health information by using an Internet-based electronic application called MyChart. In order to obtain access to MyChart, you are required to acknowledge that you have read these terms and conditions and find them acceptable. These terms and conditions also apply when you are viewing another adult or child's health information as an approved proxy.
Privacy - The privacy of your health information is extremely important to us. McFarland Clinic PC and Mary Greeley Medical Center will maintain your health information in strict confidence and will not disclose it to any unaffiliated third party unless you authorize that person to receive your health information or it is permitted to be disclosed by law. Please review the McFarland Clinic and Mary Greeley Medical Center Privacy Notices for an explanation of how, when, and why we use and disclose your health information.
Use/Access - MyChart should never be used for urgent matters. For all urgent medical matters, contact your physician's office by phone, and/or go to the emergency department of a local hospital, and/or dial 911.
MyChart is intended for the use of patients twelve years of age and older" or "Patients twelve years of age and older may sign up for personal access to MyChart and/or proxy access.
Your MyChart account should only be used to communicate with McFarland Clinic or Mary Greeley Medical Center regarding your own health matters. It should not be used to discuss health matters of family members or others. If you need to communicate with McFarland Clinic or Mary Greeley Medical Center regarding family members or others, it is appropriate to use Proxy access for the individual.
All information in MyChart is provided "as is" without warranty of any kind and meant for use only to support your relationship with your medical provider. Your reliance on the information provided in MyChart is not a replacement for proper medical attention. The information displayed in MyChart may not be the complete medical record. Therefore, you must contact McFarland Clinic PC and Mary Greeley Medical Center directly for official and complete copies of your medical record or in regard to discrepancies with the medical information listed in your record.
MyChart may not be available to you at all times due to system failures, procedures, maintenance, or other causes beyond the control of McFarland Clinic PC and Mary Greeley Medical Center. Access is provided on an "as-is, as-available" basis and McFarland Clinic PC and Mary Greeley Medical Center do not guarantee that you will be able to access MyChart at any particular time.
McFarland Clinic PC and Mary Greeley Medical Center will seek to provide a timely response to your electronic messages. In some situations, staff that need to respond to a message may not be immediately available. Therefore, the anticipated turnaround time for a response to a patient inquiry is 1-3 business days.
Your electronic messages may be shared with the McFarland Clinic PC and Mary Greeley Medical Center staff that assists your physician in providing your medical care. However, your messages will only be available to appropriate clinical staff. If your provider is out of the office or unavailable, messages sent within MyChart may be routed to other appropriate and authorized caregivers within McFarland Clinic PC and Mary Greeley Medical Center in order to facilitate a timely response to your request. In addition, McFarland Clinic PC and Mary Greeley Medical Center are only able to respond to electronic messages based on the information provided by you. Therefore, if there is insufficient information provided, McFarland Clinic PC and Mary Greeley Medical Center will be unable to provide accurate and reliable services.
Any activities within MyChart may be tracked by computer audit and entries made may become part of your medical record.
Inappropriate Use/Termination/Death - MyChart access is granted for the purpose of furthering your medical care. Use of MyChart unrelated to this purpose may result in the suspension or termination of access privileges. Improper use includes, but is not limited to, the use of inappropriate, threatening or abusive language, requesting appointment times that are not needed or are frequently cancelled, and any other use that McFarland Clinic PC and Mary Greeley Medical Center determine in their sole discretion constitutes a disruption to McFarland Clinic PC and Mary Greeley Medical Center operations. In the event of a death, MyChart and proxy access will be deactivated. McFarland Clinic PC and Mary Greeley Medical Center reserve the right, in their sole discretion, to terminate a user's access to all or part of the MyChart website, with or without notice.
MyChart ID and Password - You will be provided with an access code when you initially enroll in MyChart. You must use the access code that is given to you to create a unique personal login name (called "MyChart ID") and password. Once the access code has been used, it will no longer be valid. The login name and password you choose will be used to access your health information in MyChart and are unique codes that identify you in the MyChart system. Any inquiries and entries you make in MyChart will be logged with your identity and may become part of your official medical record. Therefore, it is extremely important that you keep your login name and password completely confidential. Anyone with access to your login name and password will be able to access your health information, as well as read your messages and send new messages as if they were you. It is your responsibility to prevent disclosure of your login name and password, and to change your password if you feel that the security of your password has been compromised. You may change your password at any time.
Verification of Identity - Your enrollment in MyChart is contingent on verification of your identity by a McFarland Clinic PC or Mary Greeley Medical Center employee.
Secure Communications - All communication between you and your McFarland Clinic PC and Mary Greeley Medical Center providers occur over a secure connection. However, if you elect to receive Internet e-mail messaging notifying you that new medical information is available in your MyChart account, please take the following into consideration. Although these e-mail messages will not contain your medical information, the notification that new medical information is available by accessing MyChart may be information that you do not want others to know. All of the health information available to you is accessed within your MyChart account and is protected and securely maintained by McFarland Clinic PC and Mary Greeley Medical Center. You may download, use and copy materials from the MyChart website for your personal, noncommercial use only, provided all copies include our copyright information and logo, and the information is not altered in any way. Except as authorized in this paragraph, you are not being granted a license under any copyright, trademark, patent, or other intellectual property right in the material or the products, services, processes or technology described in this website.
